Assessment for Education, Health and Care Plans: Guidelines

Education, Health and Care Plans (EHC) are designed to support children and young people with long term and complex needs that are impacting on children and young people’s ability to access learning and develop independence.

In Stoke-on-Trent, we have taken the opportunity of the changes in SEN legislation from September 2014 to ‘refresh’ our guidelines to ensure clarity and openness about what children and young people will gain from an EHC Plan.

Our guidelines reflect the four main areas of need, outlined in the 2014 Code of Practice:

  • Cognition and Learning
  • Communication and Interaction
  • Social, Emotional and Mental Health
  • Sensory and/or physical needs

The guidelines ask professionals requesting an EHC Assessment to submit evidence not only about the child’s needs but about:

  • Working with the child or young person  and their family to ensure their views are heard and recorded
  • Interventions and support that has been put in place by the child or young person’s setting
  • How progress has been monitored and interventions evaluated
  • What are the outcomes of targeted support and provision
  • Information on how the difficulties impact on the child or young person
  • What advice has been sought and acted on from external agencies
  • Use of Element 2 High Needs Funding (delegated)

This is to ensure that children and young people whose needs can be met through their setting are appropriately met and wider services involved as necessary.  SEN Services in Stoke-on-Trent are committed to ensuring we have the fullest possible understanding of a child or young person and their environment in order to appropriately consider requests for an EHC Assessment.
