Referrals are accepted via a medical professional such as School Nurse, Health Visitor, GP, Paediatrician or Allied Health Professional. Referral forms need to be completed in full before being processed.

Referrals are accepted for Children aged between 0-16 years of age who have a physical disability or difficulty which impacts on the child/young person's ability to participate in daily activities including self care, school work tasks and play/leisure activities.
