Children and Young People Learning Disability and Autism Keyworking Service - North Staffordshire Combined Health Care - Organisations

The Stoke-on-Trent and Staffordshire Keyworking Service provides named keyworkers for children and young people (ages 0-25) with a learning disability and/or autism. The service coordinates care across health, education, and social care to prevent unnecessary mental health hospital admissions (Tier 4) and placement breakdowns. The Keyworking service advocates for children and young people and their parent carers and supports them to navigate the systems of support that they need to access.
