Planning for independence
Parent carers can expect:
- early discussions about independence from age 14
- a transition assessment from adult social care if the person is over 16 and has an appearance of need for care and support
- regular reviews through the EHCP process, where information can be shared and discussed about transitional services and support.
It is helpful to start thinking about options early, even if your young person may stay living at home.
