Early Years Inclusive Learning Service
The Inclusive Learning Service is made up of the Early Years Forum, Portage Home Teaching Service and the Hazel Trees Education Nursery. The service works in partnership with parents/carers, schools and setting to deliver a high quality service for preschool children aged 0 - 5 with additional needs in Stoke-on-Trent.

Early Years Forum
The Early Years Forum is a monthly meeting of professionals who work for a range of agencies that provide services for young children. The Early Years Forum is for pre-school children aged 0 - 5 with additional needs.
Portage - Home Teaching Service
Portage is a home based teaching service for pre-school children aged 0 - 5 with additional needs who are not attending an Early Years setting. Parents are an important part of Portage visits and work with the Portage home visitor to set targets for the child and then plan activities to help their child reach these targets.
Portage Home Visitors work with the child and their families in the home or local Family Hub . Portage can start when a child is a few weeks old until they move into an early years setting or school. Please see the Portage page for more information.
Hazel Trees Education Nursery
The Hazel Trees Education Nursery is an assessment nursery that supports pre-school children from Stoke-on-Trent aged 2 - 5 with development needs.
Staff working in the group include a teacher and highly skilled Early Years Practitioners. In addition input is provided by professionals from a range of other services including health and social care.
Places at Hazel Trees nursery are allocated by the Early Years Forum. Please see the nursery page for more information.
